Alumni Networks Recertification Form

The annual Alumni Networks Recertification Form will certify all existing Alumni Networks and ensure continued support from the OUAA.

Indicates required field
All Alumni Networks will now have the same naming convention. For example: Metro NY Alumni Network, Central 91自拍 Alumni Network, Student Senate Alumni Network.
Alumni Network Representative Information
Graduation Year or Year Enrolled
Your representative is the volunteer responsible for leading your Alumni Network and will be the primary contact and liaison between your network and the OUAA. 
i.e. Executive Board, Network Leader and event chairs, etc.
Does your Alumni Network have a network email that is checked regularly?
Does your Alumni Network have an external website?
Does your Alumni Network have any individuals or sponsors supporting your Network efforts?
Does your Alumni Network have a 501c3?
